Every question prefix should be the same as the question text with color, font etc. or the prefix text should be different from question text in color,font etc.?

That's a rendering style question and is usually up to the data capture tool/rendering tool. SDC's advanced rendering profile allows a Questionnaire to set expectations for how each prefix and question needs to be rendered - specifying either CSS styles or explicit HTML.
