We are planning to represent symptoms as observations. What is the right observation-category to use? Would it be 'survey'? Also, for risk factors like smoking would you use 'social-history'?

I see US core uses social-history for smoking status: https://www.hl7.org/fhir/us/core/StructureDefinition-us-core-smokingstatus.html

I think 'exam' would be most appropriate for symptoms, though it's not perfect. Note that the value set is only 'preferred', which means your free to use other codes. You can also submit a change request if you feel there's a gap in the set of recommended codes.

Thanks @Lloyd McKenzie - follow-up question. Where/how are the observation category codes defined in a Questionnaire? We are trying to work through how to extract this using DefinitionBased extraction approach. Do you know of any good examples? Thanks

They're not. Category is optional, so we hadn't thought about how to populate it, but given its importance to filtering, we probably should have. Can you submit a change request for us to talk about this?
