I know that the form software (such as LHC Forms) can influence the attachment file size accepted and that there is also an extension to limit the max file size, but outside of that, are there other limits? Does HAPI set a limit? I don't see anything at https://www.hl7.org/fhir/codesystem-item-type.html#4.3.14.220.2 or at https://www.hl7.org/fhir/questionnaire-definitions.html

There are no intrinsic limits in the standard. Servers can certainly choose to impose limits - and in some cases they may be configurable.
