I stumbled uppon the Active Questionnaire blog post by @Grahame Grieve (http://www.healthintersections.com.au/?p=2835). He mentioned reference libraries that would implement these functionalities, but i cannot find any. I can see that the lforms project is moving in this direction. Are there any other? Also, the active questionnaire functionalities are part of the Structured Data Capture (SDC) Implementation Guide. Are there any implementations of that yet?

I believe this is now supported by http://test.fhir.org/r4 - but that may be rough around the edges. I don't think it's supported anywhere else yet. And yes, it's defined by SDC. Argonaut and PRO have been experimenting with it - not sure how far we are with actual implementations. @Eric Haas @Nagesh Bashyam ?
