14838 NPI and DEA numbers field can be introduced.
DEA and NPI identifiers would be communicated in Practitioner.identifiers (and maybe also in PractitionerRole.identifiers). So you can search for these already using the identifier search parameter with the appropriate identifier system.

NPI and DEA are realm-specific identifiers. Do we want to call out realm-specific IDs in the base spec?
