Discovered while testing:
R5 defines the extension http://hl7.org/fhir/StructureDefinition/patient-animal. It's not a modifier extension
There's an implied extension http://hl7.org/fhir/1.0/StructureDefinition/extension-Patient.animal which is a modifier extension, though the validator wasn't picking that up until the next release

This may cause some confusion - this not hopefully will serve as documentation for anyone who's confused about this, but PA might consider making a note against that extension in R5 (I think it's right that it's not an extension, but clarity might help)
