It looks like you have a bad XML file somewhere. You need to ensure all of your XML files are valid against their respective schemas - for example using a validating editor such as XML Spy or Oxygen.

When validating, you'll have to tell oxygen what schema to validate against - easiest is to always use fhir-single.xsd.
