My theory is because of the markdownify
filter being used in Jekyll, but I haven't had a chance to work on it and create a CR. I will be converting Da Vince DEQM to the templates and then I can chase it down later today....

funny - it works locally for me ;) also - it wouldn't hurt to repeat what the file pattern is for notes and intro files on https://build.fhir.org/ig/FHIR/ig-guidance/using-templates.html#page-files

@Lloyd McKenzie this is an issue in the base template - include doesn't process .md. See https://stackoverflow.com/questions/41966638/jekyll-on-github-pages-include-markdown-in-another-markdown-file
