When creating a profile, is it permitted to remove the summary flag from an element? For example, could a profile on Observation change issued.isSummary to false? I couldn't find anything that explicitly allows or prohibits this in the flag-related documentation or the description of ElementDefinition.isSummary.

no it's not valid. the summary flag is not subject to profiling
