Stream: IG creation

In the generated terminology bindings table, the Name column seems to always have the value of ?ext for every binding. I saw this in more than just my IG, so I think it may be a more general problem. A few example pages showing it:

ok I sorted this out. While doing that, @Chris Moesel, I noticed that you have some erroneous bindings, to http://loinc.org. That's a code system, not a value set. You need http://loinc.org/vs

Thanks, Grahame. Yep -- I'm well aware of it. In fact, I had already logged our own bug against it. I think someone fixed it but it hasn't been pushed yet.
