Within PCWG for the past month or so we have been discussing the concept of "ownership" of "custodian" of a Care Plan and how FHIR Care Plan resource is missing this.

We also discussed, in the meantime- since the NIH NIDDK TEP feels strongly that there be a Care Plan "owner", within the MCC IG that I should create an extension for this. I have learned that though extensions in FHIR are welcomed - I see the statement,
"To make the use of extensions safe and manageable, there is strict governance applied to the definition and use of extensions. Although any implementer can define and use extensions, there is a set of requirements that must be met as part of their use and definition."
But, I am not seeing what exactly these "governance rules" and "requirements" are.

What is involved in complying with the "governance"?

well, we should do something about it. Providing some actual guidance would be better than removing language saying there should be criteria and governance
