The two errors below are felt to be due to a Questionnaire rendering issue in Publisher. We had hoped that they would resolve with the latest Publisher release, but they did not.

Interesting, I've looked into this issue much more closely, and the tree is being rendered as an R5 questionnaire, even when the source is an R4 questionnaire - the specific place this makes a difference is on the type column, where you don't see the R4 value of choice/open-choice.
