Given the state of the state of Advanced Directives or Personal Advanced Care Plans in FHIR, within the MCC eCare Plan IG I want to, via CarePlan.supportingInfo reference a DocumentReference constrained to look for CDA Personal Advance Care Plan (PACP) Document. Other than constraining DocumentReference.Type to LOINC code="81334-5" "Personal advance care plan" - are there other fields I should constrain, like masterIdnetifier or Identifier? Or is it better to keep it open just looking for the LOINC code. Also, feel free to tell me if I am off base with this approach. @Rick Geimer
