In the US Breast Cancer IG, we're seeing a number of these types of errors in the QA:

Unable to provide support for code system http://hl7.org/fhir/v3/ActCode

The odd thing is that the structure def it refers to does not contain a reference to http://hl7.org/fhir/v3/ActCode anywhere within it. I don't know where the code system reference is coming from (not to mention that it's odd that the terminology server says it can't support the ActCode system).
