US Core STU 4 has the $docref operation
The official URL for this operation definition is http://hl7.org/fhir/us/core/OperationDefinition/docref
which resolves to US Core STU 3.1.1 at http://hl7.org/fhir/us/core/STU3.1.1/OperationDefinition-docref.html ??
