but this code does exist:
This is what I have in the ig.json file - is this the correct edition?

Is there something else I'm missing?

I think you are dealing with a version issue here. I checked the International Edition and the effective_time for this concept is 31 Jul 2018 - so it's a new concept and I expect that the terminology server may not have been updated yet to the July 2018 release (or to the corresponding Australian or US edition, which incorporate the International release updates 2-3 months later). Doing a $lookup for 765107002 on tx.fhir.org/r4 returns "unable to find code".
