I'm puzzled that the IG Publisher doesn't expand this value set:

Is there something I can do to encourage its expansion?

I had a similar issue of "No Expansion for this valueset (not supported by Publication Tooling)" with a local ValueSet (A) that included codes from another local ValueSet (B). The expansion on ValueSet B did work properly though.

well, there's 2 problems. The first is that you are using http://www.nucc.org/provider-taxonomy not http://nucc.org/provider-taxonomy. The second is that you are using it in a value set that is nested 2 x deep, and so the underlying error message is obscured by other errors. I'll look into the second, but you can fix the first
