The link for the body-site-instance extension URL is resolving to http://hl7.org/fhir/STU3/null.html instead of http://hl7.org/fhir/STU3/StructureDefinition/body-site-instance

I've had similar issues with the extension links and posted here in the past: this is a typical qa error. Do you think they are related?

The link 'StructureDefinition-template-profile-on-profile.html#Patient.extension.valueCode' for "valueCode" cannot be resolved (

Perhaps. They are similar errors, but mine is resolving to an invalid url for an extension defined by FHIR, whereas yours is an extension defined by your IG. Is your link broken?
I am also getting a few errors like the following:
Unable to find Extension 'http://hl7.org/fhir/us/qicore/StructureDefinition/qicore-military-service' referenced at http://hl7.org/fhir/us/qicore/StructureDefinition/qicore-patient differential Patient.extension (military-service)
However, those links work just fine in the profile page in the IG. Perhaps I messed up with the slice name or something there...
