patient controlled analgesia - the patient has a morphine IV, possibly with a continuous infusion, but they can also push a button and get an additional bolus. I wasn't sure if each bolus would be treated as a separate SubstanceAdministration, or if it still is likely to vary across sites.

I'm not sure we have any specific examples in the specification, but I can try to create one.

i would treat each bolus as a separate MedicationAdministration (personal preference). But we had some discussion in IHE and we did not feel comfortable to always enforce that. Sometimes you just want the total amount in a given period.

i guess the easiest would be to have one admin for the continuous infusion and one for each bolus. Should be easier to implement and provides best information.
