I'm not interested in the QuestionnaireResponse but on the Consent.
I was looking for some examples related to this case, in order to understand how other people has describe it.

This is what I have in mind:
.scope= "treatment" (even I'm not totally convinced about it..)
.category = "acd"
.policyRule = local code to be assigned
.provision.type = deny|permit (or empty)
.provision.purpose = "DONAT"

with the possibility to consider in the future to use .provision.provision to better specify the type of donation agreed or denied.

In this case we have scope and category that say the same thing "advance directive".
I was wondering if category should be more specific ...
could be used instead LOINC LP173398-1 Organ donation consent ? (it is a Document.Kind)

Realize that Advance Directives have been removed from Consent for R5. It might be better for a different resource to be used in that case

Which one for R4 ?

For R4, you can still use the Consent resource with Consent.scope=adr

Thanks @David Pyke. I wasn't aware of this although I noticed that the Consent resource documentation makes a mention of this. Is there a record (e.g. meeting minutes) about this decision I can check?
