Given that in the US vendors are in the process of developing and then deploying R4, and R4 will likely be enshrined in regulation when would this new subscriptions - requiring new resources - be realistically available?

Wouldn't it better to try to work something out that can be retrofitted on existing version? Considering that subscriptions is a general need not related to how the content is represented

Our official position here has been: we need to figure out a pattern that works first, and then think about back-portability to FHIR R4.

We definitely have some some options, when it comes to back-porting -- e.g., we might say Topic discovery is out of band, and then define some custom operations that support creation of "R5-like Subscriptions".
