if you want to add intro or notes pages to a profile, simply add
structuredefinition-<yourprofile>-intro.md (or xml)
structuredefinition-<yourprofile>-notes.md (or xml)
to the pagecontent folder.

note that the IHE template does not currently support this. there is an issue to fix this, will try to get to it soon
