Has there been any discussion on how to use Questionnaires and QuestionnaireResponse in the context of MHD/XDS? Making a Q-response available via XDS is easy, but if one were to add (to XDS) a Q as a patient-related document, how would one know that the Q has been responded to? There's some workflow/status aspect that seems to be missing.

That is not the workflow that XDS is designed for. Sounds more like Request For Data (RFD). Yes there are profiles using RFD. Mostly coming from the Quality domain. Although I think they have resisted using FHIR so far.

Also, look at SDC (Structured Data Capture) work being done (separately and not compatibly, from the little I understand) in both IHE and HL7.
