Just started viewing/contributing. In London we use Manifest to carry resources that carry across all documents in a submission. Given the lack of actual live services many are are held as contained resources eg patient. Using manifest allows this to be defined only once .

That level of compacting is not very friendly to systems using the data. The tendency is to simply duplicate, that is to fill out DocumentReference completely.
