I added a FHIR connectathon track for IHE-on-FHIR, that is right now focused only on MHD. I am happy to open it up to the other IHE profiles using FHIR, but for now I wanted to focus it. Should have the IHE Connecathon tooling present.

@John Moehrke I should have TestScripts in Touchstone ready and available to execute for PDQm and PIXm before we get to Cologne.
