Most of the EHR vendors are supporting DSTU2 (I've tested with Epic, Allscripts and Cerner). So this requires using fhirclient 1.0.3, but that version does not support app_secret. It looks like support for app_secret was added in 1.0.6 - so should I be using that version? It's not mentioned in the README, it says to use 1.0.3 for DSTU2.
