I included 6 examples where we currently use 'custom text'. I think our examples can be roughly classified into:
1. clarifying text (e.g. specifying the total amount and total period for multi-day continuous infusions)
2. advanced timing and relationships to other actions (something we brought up yesterday as a 'topic for discussion' ...e.g. see https://chat.fhir.org/#narrow/near/71450/stream/chemotherapy.20IG/topic/Medication.20Offset
3. considerations for adjusting / changing the dose over time (titration / tapering / etc)
4. conditions and "special-case" exceptions to the recommendations given on the order template

#4 (and maybe some others as well) are probably specific to the 'definitional' space and may already be representable in a PlanDefinition, and so I think that one becomes a question about whether to somehow also include that information in the ActivityDefinition or not

Pharmacy has added a resolution to this one and indicated that dosage.text is appropriate for all of these. They will also add some examples to Dosage.additionalInstruction for the types of codes expected and add some wording to the guidance
