Sample data JSON files for our patient persona, Betsy Johnson, are available here: https://github.com/chronic-care/sample-data
Dave Carlson (Jan 12 2019 at 14:11):
I created a new HSPC sandbox instance (v3.0.1) loaded with these sample data, with an open endpoint:

Care Planning track (and all interested individuals) will meet in a breakout room from 11:00-12:30 today in the FRIO Room (Capacity 25). Terrific opportunity for cross-track collaboration, please join us!
Chris Melo (Jan 12 2019 at 16:05):
The sample data seems to contain reference to the StructureDefinition for R4 - the current published version. But I understand that this sample data is intended to only be based on STU3. For instance, the "meta.profile" elements in cc-betsy-8-teams.json is specified as "http://hl7.org/fhir/StructureDefinition/CareTeam". Do these need to be updated?
Dave Carlson (Jan 12 2019 at 21:46):
Hmm, thanks Chris. Interesting question, is there a canonical URI for structure definitions that is not specific to FHIR version?
Chris Melo (Jan 12 2019 at 22:10):
Per the R4 specification, it looks like the meta.profile element needs to be "http://hl7.org/fhir/3.0/StructureDefinition/Patient" to explicitly reference STU3.
Chris Melo (Jan 12 2019 at 22:21):
BTW, this is as per http://hl7.org/fhir/versioning.html#mp-version
Dave Carlson (Jan 13 2019 at 19:20):
Care Planning track will meet in a breakout room this afternoon, 2:00 – 3:00 Live Oak Room (Capactiy 45). All interested individuals are welcome to join us for a debrief on our successful weekend! PlanDefinition standards for clinical practice guidelines, used to create new CarePlans, shared via an integration server with the care team.
Dave Carlson (May 04 2019 at 13:27):
Care Planning track will meet in breakout room Salon 1 (second floor) from 10:00 - 11:00 today. This track integrates topics of interest to many other tracks and all connectathon participants are welcome to attend and learn/contribute. Our wiki page is https://confluence.hl7.org/display/FHIR/2019-05+Care+Planning+Track
Dave Carlson (May 04 2019 at 13:31):
Care Planning topics this weekend include review and testing of the draft HL7 CPG-on-FHIR IG -- Clinical Practice Guidelines on FHIR, plus relationship with IHE profile for dynamic care plans. These IGs document patterns for using PlanDefinition, ActivityDefinition and CQL for practice guidelines, and using those definitions to create care plans and manage care for chronic conditions.
Emma Jones (May 04 2019 at 13:36):
Overview of IHE Dynamic Care Planning (DCP) Profile - describes the FHIR constructs used in this track - https://wiki.ihe.net/index.php/Dynamic_Care_Planning
The Profile is here - http://ihe.net/uploadedFiles/Documents/PCC/IHE_PCC_Suppl_DCP.pdf
Most recent update to the DCP Profile is to dynamically include creation and update of care teams/care team members while care planning.
Dave Carlson (May 04 2019 at 13:39):
Current very early draft for HL7 CPG-on-FHIR IG is here: http://build.fhir.org/ig/cqframework/cdc-acg/index.html, and will be migrated to an HL7 FHIR IG template and GitHub site soon, expected this week (thanks, @Bryn Rhodes )
Dave Carlson (May 04 2019 at 17:56):
Elsevier updated their FHIR endpoint to R4, allowing query of clinical practice guidelines from their knowledge base exported as FHIR PlanDefinition and ActivityDefinition resources. For example, this query uses SNOMED code for Chronic Kidney Disease to retrieve CKD plan definition: http://himss19.elsevierfhir.com/fhir/r4/PlanDefinition?context-type=709044004
Dave Carlson (May 04 2019 at 17:58):
(Note, this search should be changed to 'context' instead of 'context-type')
Dave Carlson (May 04 2019 at 18:00):
Using the sample dataset for this connectathon track loaded into an HSPC server, you can execute the same query as: https://api-v8-r4.hspconsortium.org/CarePlanningR4/open/PlanDefinition?context=709044004
Dave Carlson (May 04 2019 at 18:01):
These sample data will be aligned with those from Elsevier (and others) and included as part of the CPG-on-FHIR IG. Sample data sources are here: https://github.com/chronic-care/sample-data
Dave Carlson (May 05 2019 at 17:57):
Care Planning track will meet in breakout room Salon 5 at 2:00 today (15 minutes from now). Will review accomplishments from this weekend and plan priorities for September connectathon to include use of CQL with plan definitions.
Dave Carlson (Sep 14 2019 at 11:02):
Care Planning track will meet this morning in breakout room M104 from 10:00 - 11:00 am. This track focuses on an integrative use case planning and managing care for patients with multiple chronic conditions, all connectathon participants are welcome to attend and learn/contribute.
Dave Carlson (Sep 14 2019 at 11:05):
Topics this weekend include testing the HL7 CPG-on-FHIR IG -- Clinical Practice Guidelines on FHIR -- that documents patterns for using PlanDefinition, ActivityDefinition and CQL for practice guidelines, and using those definitions to create care plans and manage care for chronic conditions.

Andy Donnell (Sep 14 2019 at 15:04):
To follow up on this morning's (9/14/2014) breakout session - I'm Andy Donnell from Elsevier. I'd love to chat further with anyone regarding how to map nursing plans of care with the FHIR spec. I'll be situated somewhere amongst the Care Planning and Management tables throughout the weekend.
John Timm (Sep 14 2019 at 15:57):
@Dave Carlson Here are the coordinates for the IBM FHIR server endpoint:

Andy Donnell (Sep 14 2019 at 20:43):
Is anyone else working specifically around nursing documents?
Dave Carlson (Sep 15 2019 at 12:52):
Care Planning track, please add your name to the attendance list at https://confluence.hl7.org/display/FHIR/Attendance+Page
Becky Gradl (Sep 15 2019 at 14:12):
This might be a little late, but if anyone is interested in testing out the NutritionOrder resource (https://www.hl7.org/fhir/nutritionorder.html) or the new NutritionIntake resource (http://build.fhir.org/nutritionintake.html) I'd love to hear your feedback on it. I'm available to provide guidance on them.
