Nothing to do with the missing code, but it is interesting that in the absence of a url
parameter, the server falls through to validating against the all codes ValueSet for the CodeSystem

The NZ Ministry of Health. I had to sign a Confidentiality Undertaking with the Ministry under New Zealand's AR-DRG Classification System Licence Agreement. I was directed to them by IHPA. The Ministry then provided me with the current ICD-10-AM Edition used in New Zealand - ICD-10-AM Eleventh Edition, implemented 1 July 2019.

Yes, this is part of the general problem with ICD-10 - I63.9 isn't presently in the list of supported codes. At the present time we've been adding codes by request as needed - and we can add this one, too, if that will be of help. If we can work out a solution similar to what Peter has with ICD-10-AM, we'll be much better off.

I know I keep saying it and it has kept getting delayed, but hopefully within the next couple of days we will actually have the full list of codes available.

I think that the "grouping header" codes (e.g. 'E00-E89') will probably go away in the full representation, as they are conveniences for navigation and aren't actually "real" codes.

You might try creating a ValueSet defined as "all codes" in ICD-10-CM (by specifying only compose.include.system) and posting and expanding that on tx.fhir.org. It's a little cumbersome, but I think that should work.

Indeed, they are not proper ICD-10 codes, but it would be better to keep those block codes ("E00-E89") if possible to meet navigation/grouping requirements.

In my my experience a range of codes like E00-E89 is treated as an ICD code that is abstract in most terminology servers. So I'd agree with "not proper" if that is interpreted as =abstract.

The current (2021) complete set of ICD-10-CM codes should be available on tx.fhir.org now. Apologies to everyone that this took so long, but Grahame had server updates to work through and there were a lot of steps that had to happen. Please verify that the codes that you need are there (they should be). The hierarchical (nesting) relationships are there and should work, too (I haven't tested that yet).

That code is definitely in the value set (https://phinvads.cdc.gov/vads/ViewValueSet.action?id=87B8CF66-0F85-4CC7-BBE7-D6FBD32FD181) and I checked the PhinVads package and it's in that downloaded package.

@Sarah Gaunt The US standard for ICD-10 for billing and reimbursement and other areas, of course, is ICD-10-CM. This Cause of Fetal Death reporting is the only case in the US that I'm aware of which does use the "base" WHO ICD-10 code system. Right now, as noted ealier in the thread, tx.fhir.org does now have support for the full 1CD-10-CM code system. But it has only quite limited support for the WHO ICD-10 content, and there hasn't been a clear need and priority noted for adding that previously. I understand that there is a convertor tool available which reads the available ClaML distribution file that we should be able to use to make this available. Right now it is possible to validate the P02.1
code with ICD-10-CM on tx.fhir.org: http://tx.fhir.org/r4/CodeSystem/$lookup?system=http://hl7.org/fhir/sid/icd-10-cm&code=P02.1
. If you are able to make use of that, it could provide an immediate (even if possibly temporary) solution. I will also further investigate when we can provide WHO ICD-10 support on tx.fhir.org.

I don't think that work around will work for the IGs. The problem is the package that is downloaded from PhinVads specifies icd-10 (not icd-10-cm).
It makes my Condition example error, which in turn makes my Composition error, which in turn makes my Bundle error.

No, it isn't. But it actually wasn't that clear that it was needed until now. But looking at the reference to "ICD-10-Mortality Fetal Manual (2016)" is what makes it clear (at least to me) that the "base" WHO ICD-10 is what is used for this.
