It seems that the download links were removed from the Download Area page (https://apps.who.int/classifications/apps/icd/ClassificationDownloadNR/list.htm) by WHO.

In case it's useful, we have this CLAML -> FHIR converter https://github.com/aehrc/fhir-claml
