Hello guys, How your services manages new code systems and valuesets. Are your terminology services interpret FHIR ValueSet's as is on fly? Or in other words can i load my ValueSet into service and it will work out of the box? What kind of restrictions?

depends on the service. Several of the servers entirely support value sets on the fly

Yes, Ontoserver supports this. Likewise with CodeSystem and ConceptMap (with a few exceptions, e.g. you can't create a ValueSet whose URI starts with http://snomed.info or http://loinc.org)

You can even/also use operations like $expand without loading the ValueSet into the server (i.e. by POST-ing it as a parameter to the expand request)

@Grahame Grieve does specification assume that terminology service should support interactive creation and interpretation of ValueSets and CodeSystems or this is out of scope yet?

Spec allows for this. Not all servers support it. The new TerminologyCapabilities resource allows servers to document this
