Here is a link to SCT International license fees per practice/clinic/hospital. Interesting to note that there is no fee for low income countries.
Would the SNOMED CT data files be downloaded to some sort of Terminology database? Would this be part of the EMR/EHR database or could the Terminology data files be held on a FHIR server?

sct would typically be downloaded and implemented on a local terminology server. There are a number of good open source / free / commercial options
