Looking at the terminology resources, I'm wondering why they don't carry publishing metadata like approvalDate, lastReviewDate, and effectivePeriod?

We should probably have standard extensions for those things. But from an 80% perspective, we're not thinking that most systems will treat these (or StructureDefinitions or other artifacts) as curated objects.
