Topic: Observation Codes
Grahame Grieve (Dec 01 2020 at 10:33):
Hey does anyone know whether there's a code in LOINC or SNOMED for how long you can hold your breath for? This is being used as a clinical symptom in a suite of covid risk factors, but no obvious code leapt out at me...
Daniel Karlsson (Dec 01 2020 at 10:35):

No SNOMED found (so far)
Grahame Grieve (Dec 01 2020 at 10:36):
thanks! How stupid of me not to think of searching for Apnea....
Daniel Karlsson (Dec 01 2020 at 10:36):
I guess the term "apnea" might be ambiguous though, is it voluntary or not
Grahame Grieve (Dec 01 2020 at 10:37):
right. Is average or longest? the definition isn't really clear
Lin Zhang (Dec 01 2020 at 14:57):
Generally, apnea means pathological/passive suspension or absence of breathing while holding breath is voluntary.
Lin Zhang (Dec 01 2020 at 14:58):
No LOINC codes for the latter is found for now.
Lin Zhang (Dec 01 2020 at 15:06):
There are some loosely related finding concepts such as:
713013008 |Breath holding spell (finding)|
90091006 |Breath holding with temper (finding)|
Lin Zhang (Dec 01 2020 at 15:17):
Related LOINC Code example:
Apnea duration

those 2 snomd ct codes look like about angry children to me. See https://www.everythingasterix.com/news-and-views-content/asterix-world-cup-quarter-final-2
Grahame Grieve (Dec 01 2020 at 23:03):
oh for some definitions....
Robert McClure (Dec 02 2020 at 02:27):
To me this sounds like a LOINC observable, not a finding in SNOMED CT. I agree, LOINC doesn't seem to have anything that works for this (there are some vent settings..) I suspect this is a LOINC request. @Grahame Grieve , you going to do that? Also agree this is not apnea
Grahame Grieve (Dec 02 2020 at 03:56):
I may do it. I don't have time right now
Grahame Grieve (Dec 02 2020 at 03:58):
I'm not fussed whether it's LOINC or SNOMED CT. The particular project doing this has a long list of observations some of which are extremely clinical and way outside LOINC's current scope, where as there are obvious SCT codes, and some are very much LOINC space. Others - like this one - are in between
Robert McClure (Dec 02 2020 at 13:15):
The border between those two, and making up our own, is murky at best. But this one, given the situation, I think should be a LOINC. Something like "Breath holding capability" measured in seconds?
Lin Zhang (Dec 02 2020 at 13:43):
Its FSN might be:
Longest duration of breath holding:Time:Pt:Respiratory system:Qn:[Methodless].
Lin Zhang (Dec 02 2020 at 13:43):
There is an agreement between these SDOs to avoid duplicate efforts and enhance content consistency.
Grahame Grieve (Dec 02 2020 at 20:27):
"Breath holding capability" measured in seconds?
yep that one.
