Which will affect existing prod deployments (DSTU 2 + ?). How does one go about handling something like this? IE: can I use a new med-rt system in an older server, should clients/servers treat ndf-rt and med-rt as essentially equivalent?

yes, this is supposed to be happening (I don't know if it actually has yet)
med-rt is certainly the logical successor to ndf-rt, but I think there are likely to be enough differences that it won't be just a seamless change over - it does need some further investigation

unless they start changing the meaning codes (hope not) it does not change the system URL. We do need to add a note about this on the NDF-RT page, and change it's name

an analogy here would be SNOMED RT and SNOMED CT (not that it actually matters now) - are they the same code system, or different code systems?
none of the existing meanings changed when the transition was made by IHTSDO, but IHTSDO treated them as distinct for publication and other purposes
