I think it should be revisited or only partially applied. As discussed on a recent FHIR-I call sub-national (state licenses) stuff should not go into the core international specification. (maybe US Core temporarily.) and Actually I think an external national registry is more appropriate for all this long term

We need to decide how this should and will work long(er) term - we have other similar requests from the US and elsewhere. It seems like the additional items here (Medicare number was already applied) would most likely be candidates for US Core.
