@Rob Hausam apropos the conversation on the SNOMED on FHIR call (https://confluence.ihtsdotools.org/pages/viewpage.action?pageId=106700966) about cross-CodeSystem $subsumes and whether ConceptMaps can be involved, this is what the spec says about the definition of $subsumes:
Test the subsumption relationship between code/Coding A and code/Coding B given the semantics of subsumption in the underlying code system (see hierarchyMeaning).
I've not been able to find reference to mappings in a ConceptMap affecting the behaviour of the $subsumes operation.

I can't find anything on it, either. So whether or not I've actually come across it before (maybe I made it up?) I think we can assume that it doesn't officially exist and it's not an issue.

the spec purposely doesn't get into this. A terminology server may choose to be informed by concept maps, or not, but in general a server should not assume that any old concept map is suitable for this task
