Does anyone know what the market is like for commercial SNOMED aware terminology servers? Are there companies selling products like this or is it a case of rolling your own?

At this stage, 'my' Terminology Server http://its.patientsfirst.org.nz/RestService.svc/Terminz is really just an exemplar for NZ to consider this as an approach to implementing SCT. However, it's available for the FHIR Community to use as test instance - although, for various reasons, it's currently a read-only service.

@Ewout Kramer yes to Ontoserver, but not the :8080 page -- that is the old non-FHIR version. The commercial version is https://ontoserver.csiro.au and there's a public sandbox to play with at https://ontoserver.csiro.au/stu3-latest
