The ValueSet http://hl7.org/fhir/ValueSet/@all is meant to be _"All value sets known to the server"_ but what if some of those ValueSets cannot be expanded (e.g., due to an unknown CodeSystem)?

I didn't bother trying to think about #expanding that value set without a text filter. And I ignore invalid value sets. It's intended to be loose.

Fair enough. Would it be reasonable to instead interpret it as "All codes in all CodeSystems known to the server"? The reasoning being that every CodeSystem has (should have) a ValueSet that is "all codes in the CodeSystem" and thus the union of these gives "all codes in all known CodeSystems".
