Is there any reason why ObservationDefinition, SpecimenDefinition are not under the 12.5.1.1 Definitions group? Only because they are FMM 0 or for some other reason?

ObservationDefinition should be. SpecimentDefinition is defining a 'thing', not an action, so it wouldn't be appropriate. Can you submit a change request to get ObservationDefinition properly categorized?
