Richard, all the topic specific profiles in there all import the header profile. It is just declared as a US-Realm header profile http://hl7.org/fhir/us/ccda/StructureDefinition-CCDA-on-FHIR-US-Realm-Header.html

likely because the USA has some constraints that the rest of the world doesn't agree with and finds offensive. (hard to argue with that...)
