There are cases where a provider has multiple NPI dates ( NPIDeactivationDate, NPIReactivationDate and ProviderEnumerationDate) what do you suggest we use for "start" and "end"? image.png

Probably multiple identifier: one with end date corresponding to NPIDeactivationDate, one with start date corresponding to NPIReactivationDate etc.

The start and end period for an identifier is not the same as NPIDeactivationDate or ProviderEnumerationDate. The start and end period indicate when the identifier applies to an entity.

I do not see a good place to store these descriptions of an NPI. Is this interoperability related or do you need this for a specific use case? If you do not need to be interoperable @Michele Mottini suggestion for multiple identifiers will work. I view this as registration/provider directory-type information that would not apply to all identifiers (other people will use start period and end period differently, even for an NPI).

https://www.hl7.org/fhir/contract.html seems to have everything needed (but a bit much for the few things you mentioned tracking).
