This morning we discussed mapping MSH-23 to a proposed extention, MessageHeader.#ext-receiverResponsible(Organization). I wonder if it makes sense to propose a change to the base resource, as MessageHeader may have a bright future supporting lots of v2 to FHIR transforms. What is the sense here, that an extension or resource change request is more appropriate?
