For resource DetectedIssue, can I reference PractitionerRole instead of Practitioner for element author? PractitionerRole contains both organization and location whereas Practitioner doesn't. Am trying to avoid using extension. Please advise, thanks.

For R3, not without using an extension - which is a problem with a lot of R3 resources. For R4, that should be true but looks like it's not. I'm going to see if we can sneak that fix in before R4 ships.

What if I apply referencing PractitionerRole for "implicated" element in DetectedIssue? This is in the context of providing organization and location.

That wouldn't be appropriate. "implicated" would say that the PractitionerRole was a cause for the DetectedIssue, which isn't correct. There's no issue using extensions when they're needed. In R3, if you need to reference PractitionerRole instead of Practitioner, extension is generally the correct way to do it.

Then you'd be looking at doseStatus and doseStatusReason, not status
