Hello,
For Consent.except.type, there is a code list of 2 items--PERMIT or DENY. The corresponding code list in the HCIM/ZIB for Behandelaawijzing/Treatment Directive consists of 3 items - Ja, JA_MAAR, and Nee. Subject matter experts at Epic hospitals raised that it seems problematic to map JA_MAAR to DENY. Is there motivation behind the reasoning to have it mapped this way?

The short answer is: the zib does not necessarily care about FHIR. FHIR is just another technical way to implement the zib. Tomorrow there might be "yats" Yes Another Technical Standard.
JA_MAAR is PERMIT + Consent.except. The comment for Consent.except says to that end:
If a Consent.except is used in accordance with the HCIM TreatmentDirective, then the only time an except is in order, is when the value of the element NL-CM:2.1.4 TreatmentPermitted is YES_BUT/JA_MAAR.
