In the HCIM for AllergyIntolerance, the status of an allergy (active, completed, and obsolete) is modelled in 2 elements--AllergyIntolerance.clinicalStatus and AllergyIntolerance.clinicalStatus.extension:AllergieStatusCodelijst. Is there a reason that there is a specific extension for this? Is there additional value (now, or in the future) that a Dutch-specific extension will provide for conveying allergy status? Otherwise, this seems like an opportunity to streamline the profile.

The HCIM probably mandates the use of a different coding system than the coding system used by the FHIR resource. Both may contain the very same concepts, but they're two different coding systems. For conformance reasons one has to have the extension.

Generically speaking we have consistently tried to keep notion of the HCIM codes wherever FHIR required its own vocabulary.
