is there a way to specify a sort of voc binding for the identifer.system ? That is the system element has to be valorized with one of the uri included in that list ?

my intention would be that of defining a regional patient.identifier slice where the system can be one of the 20-ish specified...(if possible)

Yes, you can define a value set for elements of type uri in the same manner as you can for strings
