Lookig for some advice on the right way to extend content of an extensible terminolgy binding - do I just create a new valueset that includes the original valueset concepts and add some more?
My particular case is a profile on Identifer.type where http://build.fhir.org/valueset-identifier-type.html is extended to include all of http://hl7.org/fhir/ValueSet/v2-0203 + some other concepts (local)

we haven't enforced any check yet - not sure that we can. You'll just have to get it right ;-)
