In our IG, some of the Profiles have actually been accepted by the government, others are approved by the community but not (yet) by the government.
How can I distinguish them? StructureDefinition.keyword? or an extension?

the distinction you are looking for is external to the IG. thus to update the IG with that external distinction is to revise the IG, where the revised IG is not the version referenced externally. Best to track that external to the IG.

I could maintain 2 external Lists of resources, each of them with the "already approved" status

If your community is your local affiliate, you may call it affiliate localization or affiliate IG. Same problem in Germany. We have several orgs or initiatives who create their own IG - as a separate websites. So, using FHIR is good, but without an overarching strategy.. ..
Would need profile hierarchies and a network of profiles working together...
