I'd like to create some scenarios with the scenario builder, but they require profiles/extensions that clinical genomics already created (e.g., DiagnositcReport-Profile for HLA Genotyping Results, Observation-Genetics). I understand clinfhir has limitations with supporting profiles in the scenario builder. What is the best approach to deal with this? recreate them in clinFHIR and then use them?

Could you try them out and see what happens? (They should be selectable if they are loaded in the conformance server you have selected). It may be possible to fix in a reasonable time frame. An alternative is to create a logical model and use that. It's not a *real* profile, but will have all the information you're trying to represent and can be used as the basis for building a profile...

Yes, I tried them and get a bunch of pop-ups. eg, when I select Observation-genetics, I get a messages like this which seem to be about the extensions
makeProfileDisplayFromProfile: Error retrieving http://hl7.org/fhir/StructureDefinition/observation-geneticsSequence {"msg":"No matching profile (http://hl7.org/fhir/StructureDefinition/observation-geneticsSequence) found on http://fhirtest.uhn.ca/baseDstu3/StructureDefinition (but not a server error - just not present)"}

means the the extension definition isn't on the conformance server. I'll take a look a bit later...
